WHITELAND, Ind. – The Clark-Pleasant School Corporation is hosting an optional immunization clinic for students at Whiteland Community High School North Building on Tuesday, July 23, from 10 a.m. to 1 p.m. The Indiana Immunization Coalition (IIC) will be onsite as it “breaks down barriers and brings vaccines to you.”
Consent forms, other clinic documentation, and the registration form for the clinic can be found at https://bit.ly/45aLUSG. Use Enrollment Code IN65942. If you need a form in a different language or assistance completing the registration, it will be available on registration day.
Students registered for the clinic will be vaccinated with all ACIP-recommended vaccinations due at the time of the clinic. This would include the HPV vaccine starting at nine years of age, influenza (September through April), and Meningitis B at 16 years of age.

For any specific vaccine refusal, parents should email clinic@vaccinateindiana.org at least 48 hours before the clinic. IIC will not initiate the COVID-19 vaccine without explicit parental request.
IIC accepts all insurance (commercial, Medicaid, and Medicare), and will provide immunizations at no cost to those without insurance.
