COLUMBUS, Ind. – Thrive Alliance recently appointed Jennifer Moffatt as Dementia Outreach Specialist. In her new role, Moffatt will implement the State of Indiana’s Dementia Care Specialist Program (DCSP) for Thrive Alliance’s PSA (Planning and Service Area).
Intending to improve Indiana’s dementia-related population health outcomes, the DCSP supports Indiana’s transformation into a dementia-capable state equipped with dementia-capable care and services for all Hoosiers. Special attention will focus on strategies to decrease health disparities concerning dementia in Indiana.
Moffatt joined Thrive Alliance in 2024 as a Care Manager after serving 10 years as a human resources generalist at companies in the Columbus area. She holds a Bachelor of Science and Education degree from IU.
The mission at Thrive Alliance is to improve the quality of life and health for people at risk of losing their independence. The agency helps people get the correct information, resources, and support needed to live independently. Their network of programs focuses on issues related to affordable housing, caregiver support and services, nutrition and wellness programs, transportation and accessibility services, and fostering community-wide support for aging-related topics.
Thrive Alliance serves as Indiana’s Agency on Aging for Bartholomew, Brown, Decatur, Jackson, and Jennings counties.
